Eligible Ages Age 14 days to 80 plus
Maximum benefit options For age 14 days to 69 - $500,000 and $1000,000
Age 70 to 79 $50,000 and Age 80 Plus $15,000
Deductible options $0, 50, $100, $250, $500, $1,000 and $2,500
Co-Insurance - claims incurred in USA or Canada For the Certificate Period, Underwriters will pay 90% of the next $20,000 of Eligible Expenses after the Deductible, then 100% to the Overall Maximum Limit.
Co-insurance - Claims incurred outside USA or Canada For the Certificate Period, Underwriters will pay 100% of Eligible Expenses after the Deductible up to the Overall Maximum Limit
Pre-Existing condition exclusion period 3 Years immediately preceding effective date. The Company will pay up to $300 per day for each night spent in the Hospital after being admitted for a heart attack or stroke. Maximum amount payable is $5,000.

Coverage can be renewed up to 48 months. Changes to address, email and or phone number can be made at the time of renewal. If changes to plan, policyt maximum or deductible or different buy-up options are required, a new application will have to made.
